Life Time Fitness® and 3M 2008 Filtrete Clean Living Sweepstakes

1. Entry. NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. The 2008 Filtrete Clean Living Sweepstakes ("Sweepstakes") begins at 12:00:01 a.m. CT on 10/1/08 and ends at 11:59:59 p.m. CT on 12/30/08. Entry forms will be available at any participating Life Time Fitness location through 11:59:59 p.m. CT on 11/30/08 or while supplies of entry forms last. You may enter the 2008 Filtrete Clean Living Sweepstakes beginning October 1, 2008 by (a) filling out an entry form in the front lobby or workout area at any participating Life Time Fitness location or (b) mailing your name, address and phone number on a 3x5 note card to: Life Time Fitness, 2902 Corporate Place, Chanhassen, MN 55317, Attn: Filtrete Clean Living Sweepstakes, or (c) complete the online entry form at www.filtrete.com/cleanlivingsweeps. Mail entries must be received by 11 p.m. Central Standard Time ("CST") on January 15, 2009. Any entries that are not filled out correctly, completely and legibly may be disqualified. Entries will only be accepted as noted above. All entries at Life Time Fitness locations become the property of Life Time Fitness and will not be acknowledged or returned. Life Time Fitness may provide information on the entry form to 3M as permitted by entrants. Life Time Fitness may terminate the Sweepstakes at any time and without notice.

2. Eligibility. You must be a legal resident of the United States and at least 18 years of age to enter and win. In Alabama and Nebraska, you must be 19 years of age to enter and win. Officers, agents, employees and representatives of Life Time Fitness, 3M and/or Gage Marketing Group and their immediate family members are not eligible for entry. The Sweepstakes is void where prohibited or restricted by law, and is subject to applicable federal, state, and local laws and regulations. No more than one (1) entry per person will be allowed.

3. Drawing. One (1) winning entry will be drawn on or about January 30, 2009. Subject to the eligibility requirements of these Official Rules, the person whose name is drawn will be awarded the prize. Participants need not be present to win. The winner will be notified by phone or mail in accordance with the information supplied.

4. Winner Affidavit of Eligibility & Release. Before receiving the prize and as a condition of receiving the prize, winner must provide valid photo identification featuring his or her name and address as it appeared on the entry supplied, meet certain creditworthiness requirements of 3M or its affiliates, provide proof of proper age and residency, proof of insurance, and sign an affidavit of eligibility and release supplied by Life Time Fitness which, among other disclaimers, (a) releases Life Time Fitness and 3M and their subsidiaries, directors, agents, agencies, affiliates, promoters, officers, directors, employees and related persons from any and all liability related to this Sweepstakes and the receipt and use of the prize, and (b) grants Life Time Fitness and 3M the right to publish the winner's name, city/state, photograph, voice, testimonial, approved biographical information, or other likeness and/or prize information or personal exposition (and/or any edited portion thereof) for promotional, advertising and/or publicity purposes in any media, now or hereafter known throughout the world in perpetuity, without compensation or notice to, or further consent of, the winner to the extent permitted by law. Winner must complete, sign and return the notarized affidavit of eligibility and release to Life Time Fitness before taking delivery of prize. Failure to do so will result in disqualification, forfeiture of the prize and selection of an alternate winner.

4. Odds. The odds of winning depend upon the number of entries received.

5. Prizes. The Sweepstakes grand prize ("Prize") includes a) four (4) roundtrip airline tickets to Miami, Florida up to $750 per person b) $650 to be used for transportation expenses, and c) a voucher for hotel accommodations in a deluxe resort view two-bedroom suite for five (5) nights at the Cheeca resort in the Florida Keys (approximate retail value $3,300). Travel must be completed between May 1, 2009 until December 1, 2009 (Blackout dates apply). Airline travel must be booked through Travel One. Prize is non-refundable, non-transferable, and subject to all other terms, conditions and restrictions of Travel One, the applicable airline and Cheeca resort. Winner shall be responsible for all other fees, taxes, surcharges, or any other incidental costs.

6. Release. By entering the Sweepstakes, each participant agrees to follow these Official Rules and to release, discharge, and hold harmless Life Time Fitness and 3M, their respective affiliates, subsidiaries, agents, together with the present and future officers, agents, employees and representatives of each of them, from any and all responsibility or liability whatsoever for any property loss, damage, personal injury, or death, in connection with the awarding, receipt, acceptance, use and/or misuse of any prize provided pursuant to the Sweepstakes and/or any person's participation in the Sweepstakes.

7. General. All interpretations of these Official Rules shall be made by Life Time Fitness, whose decisions and interpretations shall be final and binding. Life Time Fitness reserves the right to cancel, terminate or suspend this Sweepstakes or any part of this Sweepstakes for any reason. Life Time Fitness reserves the right, at its sole discretion, to disqualify any individual it reasonably suspects or believes to be acting in violation of these rules. Life Time Fitness shall not be responsible for lost, incomplete, misdirected, misdelivered, misplaced, stolen, illegible, mutilated or defaced entries or for entries that are not received by Life Time Fitness for any reason.

8. Winner List. For notification of the winners submit your request in writing by January 30, 2009 to Life Time Fitness, c/o Marketing Division, 2902 Corporate Place, Chanhassen, MN 55317. Attn: Filtrete Sweepstakes.

9. Sponsor. The sponsors are Life Time Fitness, 2902 Corporate Place, Chanhassen, MN 55317 and 3M Construction and Home Improvement Markets Division, 223-4S-002, St. Paul, MN 55144.
